Before your first visit
- •We check your insurance benefits before you come in. We're in network with BlueCross BlueShield and Medicare and accept HSA and FSA cards. Insurance and payment
- •We'll email your new patient forms after you book, or you can fill them out when you arrive.
- •Bring your insurance card, a photo ID, a list of medications, and any recent imaging reports.
- •Plan on 30 to 45 minutes. What happens at your first visit
- •Been in a car accident? Tell us when you call so we can sort out which coverage applies.
